The Registered Nurse assists in the coordination and delivery of nursing care to short stay patients in accordance with standards of practice and current federal, state and local guidelines and requirements. Guides and directs the activities of the Certified Nursing Assistants.

Responsibilities Shares responsibilities for conducting admission assessments, MDS and care plans with the Nurse Manager and the other staff nurses. Reviews, modify as needed, patients’s care plans in collaboration with patient, family/designated representative and the interdisciplinary team. * Conducts weekly wound care assessment/documentation updates. Shares responsibility in documenting acute episodic changes. Meets medicare/managed care documentation requirements. Initiates accident/incident reports and accompanying investigation as needed. Creates a resident profile for each new admission. Responds to patient/family inquires, concerns and problems on a timely basis. Communicates and report relevant and significant patient related information or changes in condition to the Physician, Nurse Practitioner and Administrative Nurse Manager. Administers IV’s/PICC line maintenance, oxygen therapy etc and documents patient’s response to care. * Administers medication as needed and documents the same. Conducts assessments for self­administration of meds within the first 2 weeks. Monitors compliance to medication administration. Reconciliates MD/NP orders and review for accuracy of orders. Ensures that orders for medications, consults and other ancillary services are carried out. Acts as liaison with Director of Case Management and ensures that updates are provided on a timely basis. Attends care planning and discharge planning review meetings. Provides patient/family education during stay and to prepare for discharge. Ensure that the rooms/beds assigned to new admissions, In­house transfer are ready to accept the patient. Conducts daily rounds with the Nurse Managers. Responds appropriately to patient care emergencies, provides guidance and direction to other caregivers in managing emergency events.
